Sentence Correction: Subject Verb Agreement - A Plural Subject Consisting of Singular Nouns that are Connected by 'and'

The cardiovascular system, which regulates blood distribution, and the lymphatic system, which controls lymph distribution, combine to form the circulatory system, responsible for the transport of gaseous, nutritional, and waste between cells.

Incorrect.

This answer choice contains a Parallelism error and is grammatically incorrect. The first two items in the list at the end of the sentence are adjectives (gaseous, nutritional) but the third item is a noun (waste). In a list, the items must be in the same part of speech and/or of the same kind of part of speech.

What helps us identify this question as a Parallelism question as well as identify the mistake is the following Stop Sign:

A list of 3 items or more, separated by commas and and/or before the last item.

Incorrect.

Although this answer choice corrects the original Parallelism error by using only nouns in the list at the end of the sentence, it is grammatically incorrect. The plural subject The cardiovascular system...and the lymphatic system does not agree with the singular verb combines.

Bravo!
